This Privacy Policy (“Policy”) explains our online information practices regarding the collection of information from this Web site that can personally identify you and/or your company, and the choices you can make about the way your information is collected and used. Personally Identifiable Information (or PII) is information, such as your name, address, telephone number, password, username, medical information, gender, ethnicity, age and e-mail address that could be used to contact you. If you provide us with personally identifiable information, we will treat it in accordance with this Policy. This Policy only covers the Web site owned by Persona. Please note that Persona web site is directed towards users who reside in the United States. Persona can only “read” cookies that are placed on your computer from our own Web site. Usage of cookies is not linked to any personally identifiable information. Protecting Your Information
The personal information you provide online is retained for no longer than is necessary, depending on the purpose for which it is held, and is electronically stored on a secure server. We protect the personal information you share with us, and we do not keep or store credit card information. Access to your personal information is limited only to those Persona employees and clinicians, all of whom are required to keep your information confidential. Persona does not sell, loan, trade or lease any personal information collected on our Web site.
To prevent unauthorized access, maintain data accuracy, and ensure proper use of your personally identifiable information, Persona and its third-party partners employ appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online, consistent with industry practices.
Children’s Privacy
We do not knowingly collect personal information from children younger than 18. If we learn that we have personally identifiable information about a child younger than 18, we will delete that information. If you are younger than 18 and wish to register to obtain further information or be included in a survey, your parent or legal guardian must register.
California Privacy Protection Act or CPPA
California law allows California residents, upon a verifiable consumer request, to request that a business that collects consumers’ Personal Information give consumers access to certain information. This includes the specific pieces and categories of Personal Information that the business has collected about the consumer, the categories of sources for that information, the business or commercial purposes for collecting the information, and the categories of third parties with which the information was shared.
California residents also have the right to submit a request for deletion of information under certain circumstances. This is not an absolute right and, in some circumstances, Persona Neurobehavior Group may decline to delete the information, such as when Persona has an on-going business relationship with you, when we have a continued need to use the information for purposes outlined in this Privacy Policy, or when we are legally required to retain the information.
Consistent with California law, if you choose to exercise your rights, we will not charge you different prices or provide different quality of services unless those differences are related to your information.
How to Submit a Request for Access to Information or Deletion of Information

After you submit the request you will be asked to verify your email address and may also be asked to provide additional information in order to verify your identity, including potentially providing a declaration under penalty of perjury. Persona Neurobehavior Group will attempt to verify your identity by asking for information that correlates with the information that we have previously collected about you. Where this is not possible, Persona may request you submit additional information for verification, such as proof of residency or redacted government-issued identification. Please note that consistent with California law, we will be unable to satisfy your request until we have verified your identity.
You may also designate an authorized agent to make a request on your behalf. In order to designate an authorized agent to make a request on your behalf, you must provide Persona Neurobehavior Group with written permission for the agent to make the request on your behalf or provide us with an executed power of attorney. You will also be required to submit your valid government issued-identification, and the authorized agent’s valid government-issued identification.
Choice to Opt-Out
If at any time, you wish to stop receiving information from us, or to have your name and other personal information removed from our web site, please reply to sender or forward the e-mail to us at
